Abstract (EN)

ABSTRACTBACKGROUND AND AIM: Hyperprolactinemia may associate with insulin resistance.This fact has been determined by many studies with methods which show insulin sensitivity.In this study we aimed to search insulin resistance with golden standard euglycemichyperinsulinemic clamp technique on hyperprolactinemic patients.SUBJECTS AND METHODS: This study was performed in Endocrinology Department ofDicle University. Sixtheen patients with microprolactinoma and 12 healthy voluntee wereincluded to the study. Fasting glucose, insulin levels and lipid parameters were measured inboth groups. HOMA-B and HOMA-IR values of groups were calculated. Euglycemichyperinsulinemic clamp technique was performed to the both group and M value of thegroups was defined. Mann-Whitney U and Chi-Square tests were used in statistically analysis.RESULTS: Age, BMI, total cholesterol, triglyerides, LDL-cholesterol, HDL-Cholesterol andfasting glucose levels of the groups were not show statisticialy difference. Basal insulin levelof hyperprolactinemic patients were higher than control group (6.85±4.68 ; 3.66±0.88 pU/mlrespectively; P<0,05). Mean HOMA-IR and HOMA-B values of patients were higher thancontrol group (1.49±1.30 ; 0.78±0.27 respectively; P<0,05) (136.28±72.53 ; 64.77±23.31respectively; P<0,05). Insulin resistance was determined on 5 patients by euglycemichyperinsulinemic clamp technique (M<4). M values of the patients were statistically lowerthan control group (5.64±2.36 ; 7.05±1.62 kg/mg/m2 respectively; P<0,05).CONCLUSIONS:1- Hyperprolactinemic patients have increased insulin resistance2- Insulin resistance of hyperprolactinemic patients is not associated with obesity.3- Increased prolactin levels may decrease releasing of interleukin-6 and adiponectinfrom fat tissue. In this way, may cause insulin resistance. Further studies are neededon this subject.4- Insulin resistance in hyperprolactinemic patients, may be result of down regulation ofinsulin receptors or a post reseptor defect in this receptors..4
