Abstract (EN)

OBJECTIVE: The aim of our study is to investigate the effects of serum anti-müllerian hormone (AMH) levels on the frequency of Müllerian anomaly and pregnancy outcomes in patients with a diagnosis of Polycystic Ovary Syndrome (PCOS) who underwent in-vitro fertilization. MATERIALS-METHODS: As of 14/10/2020, IVF cycles of patients with PCOS and unexplained infertility who applied to Bursa Uludağ University Department of Obstetrics and Gynecology, Center for Assisted Reproductive Treatments were included in our study. Demographic data of all patients, presence of signs of hyperandrogenism such as systemic disease, hirsutism, alopecia, FSH, LH and estradiol levels, serum AMH levels, uterine anomalies detected by transvaginal USG or hysterosalpingography, controlled ovarian hyperstimulation protocols and gonadotroin doses elaborated; In addition, DHEA-S, Free Testosterone, Total Testosterone and 17-OH Progesterone levels of patients with PCOS were recorded. Pregnancy results, perinatal outcomes and complications, delivery types, newborn APGAR scores were analysed. The results will be compared with patients in the control group diagnosed with unexplained infertility. RESULTS: There was no statistically significant difference between PCOS and unexplained infertility groups in terms of uterine anomaly incidence. When the subgroup analysis was examined, the serum AMH value was statistically higher in the PCOS group with uterine anomaly (n=6) compared to the patients in the unexplained infertility group (n=6) with uterine anomaly(p=0.002). The mean AMH value was 8.39 (1.97-23.47) ng/dl in the PCOS group, while it was 2.92 (1.1-8.59) ng/dl in the unexplained infertility group (p=0.002). Uterine anomaly was detected in 6 (%18,2) of 33 patients in the PCOS group. In the unexplained infertility group, uterine anomaly was found in 6 (%12) of 50 patients. Clinical pregnancy, abortion and live birth were statistically significantly higher in the PCOS group than in the unexplained infertility group. DISCUSSION: This is the first prospective study evaluating the effects of serum AMH and androgen levels on the frequency of Müllerian anomaly in PCOS patients. AMH levels were found statistically higher in the PCOS group with uterine anomaly compared to the control group. The rate of resorption anomaly was found to be high in the PCOS group.
