Comparison of spinal anesthesia with general anesthesia in gynecologic laparoscopic surgery and assessment of total oxidant levels, total antioxidant levels and oxidative stress index: A prospective controlled study

Our aim was to compare spinal anesthesia with general anesthesia in terms of safety, patient tolerance, anesthetic parameters and assessment of total oxidant, antioxidant levels and oxidative stress index.Sixty patients underwent laparoscopy, ASA I-II, between 18-45 years of age, were divided randomly into two groups. 30 patients in group I received general anesthesia and 30 patients in group II have received spinal anesthesia. Arterial and venous blood samples(pre-postoperatively) were collected, pulse rates, peripheric oxygen saturations, systolic and diastolic blood pressures, mean arterial pressures and endtidal CO2 induction were all recorded before induction, after induction, and at the 5th, 10th, 15th, 30th, 60th, 90th and 120nd minutes.Mean arterial pressures were significantly lower in group II at the 5th, 10th, 15th, 30th, 60th, 90th minutes (p<0.05). Intra group analysis for arterial blood samples pre and postoperatively in both groups were significant (p=0.000). There was more intraoperative hypotension in group II (p=0.038). Postoperative time of pain perception and analgesic consumption was higher in group I (p<0.05). Postoperative nausea-vomiting, sore throat and hoarseness was more observed in group I (p=0.000). There was no difference in terms of total oxidant, antioxidant and oxidative stress index between the groups (p values 0.849, 0.391 and 0.312, respectively).We did not detect any significant difference between oxidative stres parameters of the groups. We conclude that laparoscopy with spinal anesthesia may be a reliable, less invasive, and common sensical alternative to general anesthesia in selected cases.
