Abstract (EN)

The quality and benefit of the medical procedures are main concerns of both health care providers and patients. The main determinants of excellence in medical care are morbidity and mortality rates. Risk stratification before open cardiac surgery may give the surgeons an idea of morbidity and mortality rates. In order to calculate the morbidity and mortality risks, retrospective or prospective analysis of the patient data is of importance. Models or guidelines were developed to determine the perioperative complication risks (Kaplan ve ark., 2003). Various scoring algorithms were also developed to evaluate the perioperative risks in cardiac surgery. Parsonnet, Cleveland Clinic, French, Pons, Ontario Province Risk and EuroSCORE are well known examples of these scoring algoritms. In this retrospective analysis, we investigate the correlation between hospitalization duration of the diabetic patients who underwent coroner artery by-pass surgery and nursing care. The hospitalization duration and EuroSCORE 2 scores of the patients were extracted for the analyses. The data of the 200 patients, operated in an University Hospital at Istanbul were included in our study. Statistical analysis was performed with SPSS 24. When the intensive care and hospitalization duration was stratified according to the age groups, it was seen that there was statistically significant difference between the groups of patients with age older than 80 years and patients younger than 35 years (p<0.001). The diabetic patients who underwent coroner artery by-pass surgery were found have higher EuroSCORE risk scores and longer hospitalization time than the non-diabetic patients who underwent coroner artery by-pass surgery (p<0.001). Increasing age of the investigated patient population was shown to be related with longer hospital stay (p<0.001). Patients with renal failure, history of prior cardiac surgery, COPD and a history of significant preoperative disease was found to have hospital stay more than five days. As a conclusion, diabetic patients with advanced age who represent the high risky group should be monitored very closely in postoperative period. Meticulous nursing care is of vital importance in term of lower complication rates and shorter hospitalization duration.
