Retrospective evaluation of infants with chorioamnionide and ruptured early membrane

Abstract (EN)

Aim: Early membrane rupture (EMR) is the rupture of fetal membranes before birth begins. With EMR, there is an increase in maternal morbidity and fetal or neonatal morbidity and mortality. The most common cause of maternal complications is chorioamnionitis. The diagnosis of maternal chorioamnionitis has important effects for the evaluation and treatment of the newborn baby. In our study, it has been aimed to retrospectively examine the babies of mothers exposed to chorioamnionitis and EMR. A total of 340 patients, aged between 22-38 weeks, who were diagnosed and observed between January 2015 and January 2020 in the intensive care of the Dicle University Department of Neanotology, with an age of 22-38 weeks, were included in the study. Material and method: 199 (58.5%) of the patients were male and 141 (41.5%) were female. While 143 (42%) patients had chorioamnionitis, 197 (58%) only had EMR. 195 (57.1%) of the patients were ≤28 weeks, 145 (42.9%) were> 28 weeks old. BPD in 115 (33.5%) of patients, ROP in 30 (8.8%), NEK in 68 (20%), PDA in 109 (31.9%), 14 (4%, 1) IVK and 133 (39%) at least once episodes of hypoglycaemia were detected. While 82 (24.1%) of the screened patients were died , 258 (75.9%) were discharged. Results: In comparing infants with chorioamnionitis and only EMR, the incidence of BPD, ROP and mortality increased significantly between the two groups. The results were similar in terms of NEK, PDA, IVK and hypoglycemia attack. Birth week and birth weight were significantly lower in babies born from mothers with chorioamnionitis. Conclusion: Since there is an increase in mortality and morbidity in infants born from mothers with chorioamnionitis, the exposed patients should be monitored very closely, prophylactic broad-spectrum antibiotherapy should be initiated as soon as possible and screened carefully for possible morbidities.
