The relation between plasma GLP-1, C-RP and adiponectin levels at the obesity patients with normal and impaired glucose tolerance

Abstract (EN)
One of the most important pathophsysiological change in obese patients is systemic chronic inflammation. The aim of our study is evaluating the relation between CRP which is an inflammatory marker, antiinflammatory adiponectin and GLP-1 which is very important incretin hormone at Type-2 DM's pathogenesis in obese patients with normal and impaired glucose tolerance (IGT). We have included 60 obese patients who have BMI ≥30 kg/m2 (30 patients with normal glucose tolerance (NGT), 30 patients with IGT) and another group including 20 healty people who have normal BMI. We worked on plasma adiponectin, GLP-1, hunger insuline, lipid profile, HbA1C and CRP levels for all patients including the control group. Waist circumference of all patients was measured and insuline resistance was determined by using (HOMA). Oral glucose tolerance test (OGTT) was made to obese patients. Regarding the age and sex of the patients, the groups were quite similar. It was observed that there were significant differences in CRP, GLP-1, adiponectin levels (respectively p=0.001, p=0.001, p=0.033). As a result of comparing the obese patients which have IGT and NGT with healthy control group; we determined that at the obese patients groups which were with IGT and NGT had significantly higher CRP levels and lower GLP-1 levels than the healthy control group (respectively p=0,001, p=0,001, p=0,001, p=0,001). The adiponectin levels of the IGT obese patients' group were lower than the healthy control group (p=0.010). But there was no significant difference at the IGT obese patients' group with regard to CRP, GLP-1 and adiponectine levels. At the correlation analysis, significant positive correlation was found between CRP, BMI and waist circumference in the NGT obese patient group (respectively, r=0,427, p=0,019 and r=0,407, p=0,026). On the other hand there was no significant correlation found between GLP-1, adiponectin and the other parameters. Furthermore, at the IGT obese patient group there was significant negative correlation in terms of GLP-1 levels and waist circumference (r=-0,380, p=0,038). But there was no significant correlation with CRP levels, adiponectin and other parameters. As a result; finding high CRP levels at the obese patients, makes us think there is an inflammatory process at obesity. The observation of high CRP levels in obese cases and lower GLP-1, adiponectin levels can be used as a marker to identify IGT patients's diabetic progress in patients' follow up. To find out the positive correlation about CRP, BMI and waist circumference in NGT obese patients, in addition to the negative correlation about GLP-1 levels and waist circumference at IGT obese patients's group give support to this idea.
