The etiology and treatment outcome in pediatric traumatic cataracts

Abstract (EN)

This study was performed on 120 depending on the open and closed globe injuries traumatic cataract whom were admitted to clinic between 2007 and 2014 years. 70.8% of case were male and 29.2% of case were female. The patient age range was 1-16 and, the mean patient age was 7.16±3.56 years. Right eye and left eye injury rates were equal. Stone was the most common agent of injury (%21.7), second most common agents of injury were wooden sticks (%18.0) and knife (%18.0). Traumatic cataract occured in 65% of case were penetrating trauma and 35%of case were blunt ocular trauma. Open glob injury rate was 73.3%and close glob injury rate was 26.7%. Between the type of trauma or type of globe injury and final visual acuity were not significantly associated . Vision at admission was 30.8 % of case no found, hand movement perception in 46.7%, finger counting in 17.5 % of case, 0.1-0.4 level in 4.2 % of case and 0.5-0.7 level in 0.8% of case. Final visual acuity was 7.5% of case no found, 1.7% of case hand movement perception, finger counting in 13.3% of case, 0.1-0.4 level in 25.8% of case, 0.5-0.7 level in %15.8 of case and 08.-1.0 in 35.9% of case. IOL implantation was not done during primary reperation surgery. Primary posterior capsulotomy and anterior vitrectomy performed all of case. PMMA IOL implanted 50.8 %of case, foldable IOL implanted 41.7 %of case and scleral fixed IOL implanted 7.5%of case. The most common peroperative and early postoperative complicationis pupil regularity that was 52.5 % of case. Other complicatios are uveal reaction that was 24.2 %of case, posterior synechia was 6.7% of case, corneal edeme was 1.7 % of case glaucoma was 1.7 % of case, posterior capsule peforating was 5.8 % of case . The most common late period complication is corneal leukoma that was 68.3% of case. Other late period complications are traumatic mydriasis that was14.2 % of case, anterior synechia was 10.8 % of case, strabismus was 9.2% of case, atrophy of iris was 8.3 % of case, IOL capture was 6.7 % of case, IOL decentralization was 5.0 % of case, pigment dispersion was 5.0 % of case. Postoperative best corrected visual acuity is 77.5 of case 0.1 level or beter. The case of 51.6% is 0.5 level or beter.
