Analysis the tumor necrosis factor alpha level in peri-implant sulcular fluid

The aim of this study is to detect the tumor necrosis factor-alpha (TNF-?) level in peri-implant sulculer fluid (PISF) samples obtained from periodontally healthy 40 patients who were subjected different surgical procedures, to investigate the relationship between the clinical parameters of the periodontal disease and the percent of the early alveolar bone loss.Standard Plus (SP) with one stage system was placed in 20 patients, and Bone Level (BL) with two stage system implant was placed in 20 patients of totally 40 patients who did not have the first mandibulary molar tooth. To determine the periodontal state of the every patients, measures of modified plaque index (mPI), modified gingival index (mGI), modified bleeding index (mBI), probing pocket depth (PPD), mobility index (MI), keratinized mucosa index (KMI) were recorded, as well as peri-implant sulcular fluid was obtained and the levels of TNF-? were detected before load (0. month), after load in the 3rd and 6th months. The radiographic images from dental computed tomography which were taken before load (0. month) and after load in the 6th month were evaluated.With respect to the laboratory findings, clinical measures and the percent of the alveolar bone loss (ABL%), the 3rd and 6th month values of the alveolar bone loss, the 3rd. and 6th. month values of the PPD differed significantly between the groups. In the 3rd and 6th month, in the group of SP, means of PPD was higher than the value in the groups of BL, and this difference was statistically significant (p ? 0.05). Within group assessment, the levels of TNF-?, PPD values and ABL% increased significantly between the months 0 and 6th. In the BL group, the levels of TNF-? between 0. And 6th months, the values of mBI between 0- 3rd and 0-6th months and the volume of PISF between months 0. and 3rd significantly increased.In terms of the correlations between the levels of TNF-?, clinical parameters and ABL%, in the group of SP, in the 0. month between mPI and PPD, mGI and mBI, PISF and KMI, in the 3rd month between mPI and mGI, mPI and KMI, mGI and PPD, mBI and PISF, in the 6th. month between mBI and PPD, ABL% and mPI, positive correlation were found. In the group of BL positive correlation was determined, in the 0. month between mPI and PPD, mGI and mBI, ABL% and KMI, in the 3rd. month between mBI and PPD, in the 6th. month between mBI and PPD.As a result, this study supports the study which shows higher PPD value in the one stage surgical procedure than the surgical prosedure with two stages. When those are evaluated in the laboratory, clinically and radiografically, the presighted results can be attained in both of these two systems. Although the level of TNF-? shows increase in both of the two groups, any statistical difference could not be found between these two groups.
