Calculating the relationship between systemic immune inflammation index and disease activation in patients diagnosed with rheumatoid arthritis

Abstract (EN)
Objective: Rheumatoid Arthritis is a chronic, autoimmune, inflammatory rheumatic disease. Systemic immune inflammation index (SII) is a marker that has been shown to be a prognostic factor in inflammatory diseases. In our study, we aimed to calculate SII values in patients diagnosed with Rheumatoid Arthritis during attack and remission periods and to examine its relationship with disease activation Method: For our study, patients diagnosed with rheumatoid arthritis, between the ages of 18-75, without comorbidities, who applied to the Rheumatology Clinic of NEU Meram Medical Faculty Hospital, were identified. Those with rheumatoid arthritis activity index DAS28-ESH≥2.6 were considered to be in attack, and those <2.6 were considered to be in remission. A total of 60 patients, 30 in attack and 30 in remission, were included in the study for the study group. For the control group, 30 healthy volunteers were included. Erythrocyte sedimentation rate, C-reactive protein, neutrophil, platelet and lymphocyte values in the hemogram were analyzed in the participants and the "systemic immune inflammation index" was calculated by dividing the product of the neutrophil and platelet values by the lymphocyte values. The groups were compared in terms of the specified laboratory values and systemic immune inflammation index. SPSS version 22.0 was used in statistical analysis. p<0.05 was considered statistically significant. Results: When the demographic data of the groups included in our study were compared, no significant difference was detected in terms of age and gender. When RA attack and RA remission groups were compared, SII and NEU values were significantly higher in the RA attack group, while no significant difference was found in PLT and LYM values (p:0.000 for SII, 0.003 for NEU, p:0.191 for PLT, p:0.109 for LYM). When the RA attack group was compared with the healthy control group, SII, NEU, PLT values were statistically significantly higher in the attack group, while no significant difference was detected in LYM values (p: 0.000 for SII, p: 0.000 for NEU, p: 0.000 for PLT, p: LYM: 0.069). When the RA remission group was compared with the healthy control group, SII and PLT values were significantly higher in the remission group, while the differences between NEU and LYM values were not significant (p: 0.041 for SII, p: 0.246 for NEU, p: 0.723 for LYM, p: 0.025 for PLT). Conclusions: SII value is higher in RA attack patients than in remission and healthy population. The SII value is higher in patients in remission than in the healthy population. SII value can be used to determine disease activity in RA.
