Abstract (EN)

Objective: The aim of this study is to investigate the efficacy of single-source twinbeam technique in dual energy computed tomography by using visual mapping of the nodule, the contrast load measured in the nodule, and the concentration of iodine in the nodule. Materials and Methods: Fifty-three patients (mean age, 62 years) received 40-second single-source dual-energy chest computed tomography with a twinbeam feature after intravenous contrast application. Simultaneous scanning Images were obtained with 120kV Split Filter (120AuSn) tube voltage. The 120kVSplit Filter (120AuSn) data set and weighted average image data set data were transferred to a dual energy workstation. Weighted average image, an image of approximately 120-kV produced automatically from a combination of 140-kV and 80-kV data. The images were configured in an axial dataset with a thickness of 3 mm. Each of the 53 solitary pulmonary nodules were evaluated separately; Data sets of approximately 120-kV, which are automatically generated from a combination of 140-kV and 80-kV data, were manipulated in a special dual-energy workstation. It is a qualitative visual assessment that the manufacturer's assumed value was determined as 50% iodine uptake in the evaluation of the presence of contrast enhancement. If there was any amount of color (orange or red) in the nodule, it was considered as enhancement. Another method consisted of measuring iodine concentration. Iodine concentration was calculated in mg / ml. Results: Of the 53 pulmonary nodules, 36 were benign (67.9%) and 17 were malignant (32.1%) lung nodules. The visual assessment of contrast enhancement had 100% sensitivity and 67% specificity, respectively. The mean contrast load in benign nodules was 56 ± 152.8 (0-773) in terms of measured nodule contrast load, and 174.3 ± 237.8 (22.5-834.0) in malignant nodules. When 19.25 HU density was taken as cut-off value, sensitivity value was 100% specificity value was 66.7% and it was statistically significant (p = 0.034). In terms of iodine load of the nodule, the average iodine load in benign nodules is 3.6 ± 8.9 (0-49.9) mg / ml, and the mean in malignant nodules. It was determined as 10.3 ± 13 (1.4-51) mg / ml. When 1.35 mg / ml was taken as the cut-off value, the sensitivity value was 100% specificity value was 69.4% and it was statistically significant (p = 0.031). In terms of iodine load, the Positive Predictive Value (PPD) was 60% and the Negative Predictive Value was calculated as 100%. Contrast load PPD was calculated as 58% and NPD as 96%. Conclusion: It was concluded that DEBT will provide important contributions in the identification of nodular lesions detected in the lung as malignant and benign.
