Effects of use of dexmedetomidin and propofol on immune system at SIRS and sepsis patients

Abstract (EN)
Aim: Importance of immune system mediators and cytokines in development of SIRS and sepsis is known. We aim to study effects of use of dexmedetomidin and propofol on immune system at SIRS and sepsis patients.Material and Method: After taking approval of Faculty Ethics Committee, 40 patients with SIRS and sepsis have been included in the scope. Patients have randomly been divided into two groups; dexmedetomidin has been used as 0.3-0.7 mg/kg hour infusion at least 24 hours in group I. propofol has been used as 1-3 mg/kg hour infusion at least 24 hours in Group II. APACHE II values have been calculated before the start of sedation; values of procalsitonin, CRP, TNF alpha, IL 1, IL 6, platelet, and lactate have been reviewed. Fentanyl and morphine have been applied to patients when they need supplement analgesic. Sedation has been applied at least absolute 24 hours according to clinical condition of patient as he/she needs. Patients have been monitored during 2 days. APACHE II score have been used in follow-up of patient. APACHE II score has been estimated by using worse values within day. In addition, daily procalsitonin, CRP, TNF alpha, IL 1, IL 6, platelet, and lactate have been followed. Mortality follow-ups of patients have been made during 30 days.Results: there is statistically no significant difference among cytokine values, acute phase reactants, white sphere, platelet values, hemodynamic values and kidney function values in terms of APACHE and glaskow among groups demographically. However, negative correlation of IL-6 with SAB, DAB, and MAB has been detected in Group I. Positive correlation has been detected between IL-6 and CRP in Group I and II.Conclusion: Intergroup difference has not been detected at SIRS and sepsis patients during use of dexmedetomidin and proforol. We have concluded that these drugs have inhibited cytokine increase, since cytokine values do not increase in groups.Keywords: SIRS , sepsis, dexmedetomidin ,propofol, TNF-?, IL-1,IL-6
