Abstract (EN)
Aims: To investigate the efficiencies of the diverse lidocaine applications for the treatment of pain caused by postoperative catheters in male patients between 18 and 80 years old, which were applied urethral catheters for urological interventions. To determine the efficacy of lidocaine on the visceral pain caused by urethral catheters. Materials and Methods: This study was performed in the Anesthesiology and Reanimation Department of the Cukurova University after obtaining informed consent forms from the patients and with the approval of the ethic committee. Male patients with ASA scores between I and III, with ages between 18 and 80 and which will be operated under general anesthesia were included to the study. The patients were divided into four groups via simple randomization. In Group-1: 100 mg of lidocaine was given intravesically via the urethral catheter after completing its solution to a volume of 20cc with isotonic serum. Then, the urethral catheter was clamped and it was reopened at the end of the operation. In Group-2: Urethral catheterization was performed before beginning the operation by lubricating the catheter via cathejell ®. In Group-3: Urethral catheterization was performed before beginning the operation by lubricating the catheter via cathejell®. Additionally, 15 minutes before ending the operation, 100 mg of lidocaine was given intravesically via the urethral catheter after completing its solution to a volume of 20cc with isotonic serum. Then, the urethral catheter was clamped and it was reopened at the end of the operation. In Group-4: A lubricant without an anesthetic was used for catheter application before the operation. At the post-operative period, patients were followed in the postoperative care unit (PCU) for two hours. In the PCU, systolic and diastolic blood pressures, pulse rates, Visual Pain Scale (VPS) scores and side-effects were recorded at the 5th, 15th, 45th minutes and 1th and 2nd hours after the operation. The same parameters were also recorded in the urology ward at the 4th and 6th hours after the operation. Findings: The demographic characteristics, operation durations and hemodynamic parameters of the patients were similar between the different study groups. Group-3 was defined as the group, which feeled the pain at least. VPS scores were higher in Group I and Group II in comparison to Group III. The group, which feeled the pain at most was defined as the control group. No side effects were detected in any of the patients. Result: We concluded that the combined usage of the lidocaine gel (cathejell®) and intravesical lidocaine is more efficient to alleviate pain than the single application of these methods.
