Van Yuzuncu Yil University Dursun Odabaşi Medical Centre paediatric haematology clinic's granulocyte suspension transfusion experiment

In this study, the data of 9 patients of the age group between 0-18 who had medical treatment in Yüzüncüyıl University's Faculty of Medicine, Dursun Odabaşı Medical Centre Paediatric Haematology Clinic between 2017 and 2018, whose fever and neutrophil count were under 500/µl, who did not response to antibiotics or other treatments and therefore were subject to suspension transfusion were assessed retrospectively. To avoid alloimmunisation, granulocyte was collected from only healthy and unrelated donors. The donors had been given 5 microgram/kg dose of granulocyte colony stimulating factor (G-CSF) subcutaneously at evening 12 hours before they were given a single dose 8 milligram methyl prednisolone and apheresis orally one day before the procedure was carried out. Granulocyte was collected from donors by apheresis and the products were given to the patients by irradiating at the 25 Gy radiation dose. Before the transfusion, the average absolute neutrophil count (ANS) of patients was 52,2 cell/micromoIL and average white blood cell(WBC) count was 260 cell/micromoIL. In 24 hours after the transfusion, absolute neutrophil count and white blood cells of all patients increased. 9 patients were transfused 45 units of granulocyte suspension. Full clinical response was observed in all patients apart from one. The death rate in our study was 11.1 %. No complication was observed in the patients who were subjected to transfusion and the donors. This study has shown that granulocyte transfusion is a reliable and effective supportive treatments for children with neutropenic fever who are resistant to traditional treatments. More multi-central clinical studies need to be carried out to confirm the effectiveness of this treatment. Keywords: Granulocyte, Neutrophil,Transfusion
