Abstract (EN)
Sepsis is a life-threatening multiorgan dysfunction with high mortality resulting from a dysregulated host response to infection. Cancer patients are in the high-risk group for sepsis, as shown in many previous studies. Therefore, the recognition and management of sepsis in cancer patients is important. Due to chronic inflammation due to malignancy, biomarkers that are frequently used in clinical practice may be insufficient to diagnose infection in cancer patients. For this reason, it has become important to identify inflammatory markers which can be used in diagnose and follow-up sepsis that are inexpensive, easily accessible, have high sensitivity and specificity.. In this retrospective and single-center study, 120 patients who has both the inclusion criteria and diagnosed with sepsis considering The Third International Consensus Definitions for Sepsis and Septic Shock (Sepsis-3) criteria and 120 control patients who were followed-up for non-sepsis reasons, were divided into two subgroups as with metastasis and without metastasis. NLR, MPV, PLR values as sepsis markers were compared with other sepsis markers such as cut-off values, CRP, procalcitonin and SOFA score, which predicted sepsis in cancer patients, and whether there was a correlation between them was examined. In our study, CRP, NLR, and PLR values were found to be statistically significantly higher in the sepsis and metastatic groups. In the sepsis group, changes over time in laboratory parameters such as CRP, PCT, NLR, MPV, PLR, PLT, Bilirubin and creatinine were statistically significant. It has been determined that changes in NLR baseline values during sepsis increase the susceptibility to mortality. In line with these results, our study supports that these parameters are the parameters that can be used in determining sepsis and prognosis follow-up in patients with malignancy. In addition, it is important to know that cut-off values are higher in cancer patients than in the normal population. Unnecessary antibiotherapy should not be planned in patients with solid organ cancer who do not have clinical signs of infection, only because of the high level of inflammatory markers.
