The evaluation of relationship between preoperative and postoperative NT-proBNP levels and postoperative atrial fibrillation frequency in open heart surgery

Abstract (EN)
The aim of this study is to evaluate the relationship between preoperative Brain Natriuretic peptide (BNP) levels and atrial fibrillation frequency following in the early postoperative period, in patients who underwent coronary artery bypass graft (CABG) with cardiopulmonary bypassNT-proBNP is one of the Natriuretic Peptide that is synthesized in the heart. This hormone is released when the atrial stretch and the left ventricle pressures increases. Atrial fibrillation is a common complication following coronary artery bypass surgery. Postoperative atrial fibrillation can lead to thromboembolic events, prolonged hospital stay and increased health costs. Our objective in this study is to evaluate the effect of preoperative NT-proBNP levels on the development of atrial fibrillation after coronary surgery with cardiopulmonary bypass39 patient who had elective CABG surgery with cardiopulmonary bypass were included this study. Those who had renal failure, new onset myocardial infarction, combined valvular diseases, severe obstructive pulmonary disease in the preoperative period, patients over 80 years old and ejection fraction less than 30% were excluded from the study. Plasma NT-proBNP levels were measured and the patients were divided into two groups according to the preoperative NT-proBNP levels under 100 pg/ml and above 100 pg/ml. Plasma NT-proBNP levels were measured 5 times: T0:in the preoperative period, T1: at the end of cardiopulmonary bypass, T2:at 4. hours postoperatively, T3:12. hours postoperatively and T4:24. hours postoperatively. Patients? heart rhythms were monitored in intensive care unit and electrocardiograms were taken at two times in a day after the intensive care unitOut of 39 patients who were included in this study, 11 of them (% 28,2) developed AF in the postoperative periodTwo groups were compared according to the development of AF in the postoperative period. In Group B (which T0 NT-proBNP levels were high), the development of AF was found to be more frequent than Grup A. In Group A, 2 out of 19 patients (% 10,5) and in Group B, 9 out of 20 patients (%45) developed AF in the postoperative period. And the difference between these two groups was found to be statistically significant (p<0.05The differences between these two groups T1, T2, T3 ve T4 levels were not found to be statistically significant (p>0.05We concluded that high NT-proBNP levels in the preoperative period may lead AF postoperatively.
