Comparison of antioxidant levels in penille cavernous blood, peripheral venous blood, peyronie plaque and normal tunica albugenia tissues and investigation of the effect on peyronie plaque formation

Abstract (EN)

In this study, thiol, modified ischemia albumin, superoxide dismutase, total antioxidant, and oxidant levels will be measured in peripheral blood, penile cavernous blood, Peyronie's plaque, and normal tunica albuginea tissues of the same patient during Peyronie's surgery. Comparisons between control groups and study groups aim to illuminate the potential relationship between antioxidant deficiency and Peyronie's plaque formation. A total of 22 patients who underwent penile plication and Peyronie's plaque excision due to Peyronie's disease were included in the study. Peripheral venous blood, penile cavernous blood, Peyronie's plaque, and penile normal tunica albuginea tissues were obtained from the patients during the operation. Normal tunica albuginea tissue and peripheral blood were determined as the control group, while Peyronie's plaque and penile cavernous blood tissues were determined as the study group. Thiol, modified ischemia albumin, superoxide dismutase, total antioxidant, and oxidant levels were measured in these materials. According to the results of our study, the levels of TOS, OSI, Total SOD, and IMA ABSU were found to be significantly higher in the cavernous system compared to systemic venous circulation (p<0.001) in Peyronie's disease. On the other hand, TAS, native thiol, total thiol, and disulfide levels were significantly higher in systemic venous circulation compared to the cavernous system (p<0.05) for each. There was no significant difference between the groups in terms of D/NT100, D/TT100, and NT/TT*100 values. This indicates that oxidative stress locally increases in Peyronie's disease. Another result in our study is that the total protein levels in the lesion are significantly higher than the control tissue (p=0.014), while OSI levels are significantly lower in Peyronie's lesions (p=0.019). There was no significant difference between Peyronie's lesions and control tunica albuginea tissues in terms of TAS, TOS, and total SOD levels (each p>0.05). The reason for the reverse result of OSI levels, which were expected to increase in Peyronie's plaques, may be due to the proximity of normal tunica albuginea and Peyronie's plaques taken from the same patient during the operation. Therefore, there may be no significant difference between Peyronie's lesions and control tunica albuginea tissues in terms of TAS, TOS, and total SOD levels. In conclusion, this study examines the relationship between the biochemical and clinical characteristics of Peyronie's disease and oxidative stress, suggesting that oxidative stress may have an impact on the pathophysiology of this disease. Considering that oxidative stress can cause cellular damage and increase inflammation, these findings emphasize the potential of approaches targeting oxidative stress in the treatment of Peyronie's disease. However, further research should clarify the exact mechanisms and treatment strategies of this relationship.
