Abstract (EN)

Objective: The aim of this study is to compare and evaluate the long-term effects of Monoblock (activator) appliance, Forsus appliance and Class II elastic applications used in the treatment of Class II malocclusions on joint position and lateral cephalometric values. Materials and Methods: In our study, a total of 56 patients, including 20 Monoblock, 18 Forsus and 18 Class II elastic patient groups, were treated in Dicle University Faculty of Dentistry, Department of Orthodontics. The centric relationship (SI) – centric occlusion (SO) records of the patients were taken according to Roth's Power Centric method as the beginning of treatment (T0), the end of treatment (T1) and the follow-up period 2 years after the end of treatment (T2), and SAM records were taken according to these records. The amount of centric deviation in the dental models taken into the -2 articulator was measured using the Mandibular Position Indicator (MPI). When measuring the amount of centric deviation, deviations greater than 2 mm in the anterior-posterior and up-down directions and 0.5 mm in the transverse direction were considered as physiological limits. Likewise, cephalometric films of the patients were taken in all three periods, and dental and skeletal changes between the periods were evaluated. Results: In our study, a decrease was observed in the amount and number of physiological out-of-range centric deviations with Monoblock and Forsus appliances at the end of the treatment and during the long follow-up period. Before treatment, the dominant deviation direction was found to be back-down in all groups. In the cephalometric evaluation, the most skeletal effects were seen in the Monoblock group, followed by the Forsus group, while more dental effects were observed in the Class II elastic group. Conclusion: After the long follow-up period, it was observed that the SI-SO difference decreased in all three groups, the condyle adapted to the new position, and the centric occlusion remained stable with the improvement of interdigitation.
