Abstract (EN)
Introduction and Aim: Today, lifestyle changes are changing and shaping human behavior all over the world with globalization. Unfortunately, with this change, the frequency of diabetes has increased gradually and the concept of global pandemic has started to be used. The primary goal in diabetes should be to prevent the development of the disease. When diabetes develops, it is very important to achieve glycemic regulation in order to prevent or at least delay the macro and microvascular complications that may occur. The aim of this study was to investigate the factors that affect glycemic regulation in patients with type 2 diabetes, whether drug compliance, anxiety depression and whether or not they make a difference in type 2 diabetics with and without glycemic regulation. Materials and Methods: This thesis was designed as a case-control type study. The study was conducted between November 2018 and April 2019 at Atatürk Training and Research Hospital Endocrine outpatient clinic. The sample consisted of volunteers with type 2 diabetes who applied to endocrine polyclinics. Data were collected by using the questionnaire and Beck Anxiety Scale, Beck Depression Scale and Modified Morisky Scale. Questionnaires and scales were administered by the researcher using face to face interview method. SPSS 20 package program was used in the analyzes used in the study, p<0.05 was determined as the level of statistical significance. Results: A total of 158 subjects 84(53.2%) with glycemic regulation and 74 (46.8%) with no glycemic regulation were included in the study. While there was no significant effect of sociodemographic features on glycemic regulation (p>0.05), diabetes education (p=0.003), blood glucose measurement at home (p=0. 001) were found to be associated with glycemic regulation disorder. There was no difference between the two groups in terms of anxiety and drug compliance. However, depression scores were found to be significantly higher in the group without glycemic regulation. (p=0.012) Being diagnosed with diabetes for 15 years or longer increased the risk of poor glycemic regulation approximately 3 times. (p=0.014, OR=2,952, 95% GA [1,25-6,97] Conclusion: The main aim of the management of type 2 diabetic patients is to achieve glycemic regulation. In this respect, determination of the factors affecting glycemic regulation is of great importance. In our study, it was found that many of the factors investigated had no effect on glycemic regulation, but it was necessary to analyze and interpret more effective factors such as duration of diabetes, diabetes education and blood glucose measurement at home. In this respect, studies on diabetes, which are frequently seen in the society and whose management is under the responsibility of family physicians, should be carried out in primary health care institutions.
