Abstract (EN)

In this study our purpose was the comparison of haemodynamics, recovery and cost-effectiveness between low flow anesthesia and high flow anesthesia. 40 patients between 18-65 years with the operation risk of ASA I-II, who are undergoing septorhinoplasty and submucosal resection with the duration about 1 to 3 hours were included. Group D was the low-flow anesthesia group, and Group Y was the high-flow anesthesia group. Pulse rate, systolic arterial blood pressure, diastolic arterial blood pressure, mean arteial blood pressure, sPO2 were measured preoperatively. In the group Y the flow rate of oxygen was 3 L/min, and the flow rate of air was already 3 L/min, with total sum of 6 L/min. In the group D, 10 minutes after the induction, the flow rate was decreased to 1 L/min (oxygen 0,5 L/min, air 0,5 L/min). At 10th, 15th, 30th, 45th, 60th, 75th minutes after intubation HR, SBP, DBP, MBP, SpO2, FiDES, ETDES, FiO2, FiCO2, ETO2, Ppeak, ETCO2, MAC were measured. HR, SBP, DBP, MBP, SpO2 were measured before extubation and at 1st, 5th, 10th minutes after extubation. In group D, 10 minutes before anesthesia was finished high flow anesthesia with 6 L/min was performed.In group D values of intraoperative sPO2 at 30, 45, 60, 75 minutes were statistically lower than the values in group Y, but it was never lower than 98%. DBP and MBP at the intraoperative 15th minute were statistically decreased in group D.In group D FiO2 decreased to about 35% after the gas flow rate was decreased to 1 L/min, but it was never under the critical value 30%. The decrease in ETO2 was parallel to FİO2.In group D ETDES at 30 minutes, MAC and FiDES at 15 and 30 minutes were lower than group Y, but there was no need to change the setting of vapourisator.There were no difference between two groups in extubation, tongue exposition, orientation and Aldrete recovery times.There was no significant difference in fentanyl consumption. In group D, oxygen and desfluran consumptions were significant decreased than in group Y. In group D the desfluran consumption was decreased about 66%.KEYWORDS, low flow, desfluran, cost, recovery, haemodynamics
