Abstract (EN)
Acute Myocardial Infarction (AMI) has an important place among the causes of death all over the world. It both negatively affects the quality of life and imposes an economic burden on patients, their relatives, health systems and society. It is important to examine of AMI the effects on the quality of life and to determine the economic burden it creates by calculating its cost. In this study, it was aimed to calculate the cost of illness of AMI, to measure the quality of life of the patients, to determine the risk factors, comorbidities, and compliance with treatment in patients, and to evaluate the cost of illness and the quality of life together. The research was carried out with 284 patients who were treated for AMI in Ankara University İbni Sina Hospital in 2022. The cost of illness was calculated from the perspectives of the patient and their relatives, the reimbursement institution, and the community, using the Bottom-Up Costing approach. It was measured that disease-specific and generic quality of life. It was obtained the data patients' quality of life and on direct costs and income losses incurred by patients and their relatives by telephone interview. It was taken the medical care costs incurred by the reimbursement agency from the patient bills. It was evaluated the effect of possible uncertainties on the cost of illness with a one-way sensitivity analysis. It was made budget impact analysis of AMI. The disease-specific quality of life score of the study group was 2.22±0.90 in HeartQol physical, 1.88±0.54 in HeartQol emotional and 2.13±0.73 in HeartQol general. As the general quality of life score, EuroQol index is 0.84±0.24 and EuroQol VAS is 70.74±22.94. The total cost of illness is 1,565,307.69 TL ($94,190.67) and average cost of illness is 5,511.65 TL (331.66 USD) from patient and patient relatives perspective. The total cost of illness is 3,029,935.39 TL ($ 182,323.04) and the average cost of illness is10,668.79 TL (641.98 $) from reimbursement perspective. The total cost of the illness is 5,339,458.12 TL (321,296.03 $); the average cost of the illness is 18,800.91 TL ($1,131,32) from societal perspective. It was found that significant difference between quality of life and gender, age, employment status, day of stay, hypertension, diabetes, cholesterol, smoking and obesity risk factors. It was found that significant difference between cost of illness and obesity and diabetes risk factors. When the relationship between cost of illness and quality of life was examined, It was found that significant difference between the disease cost and the disease-specific quality of life score from the patient and patient relatives perspective. It is recommended to use of resources more efficient for reduce the medical care costs, which have a significant share in the cost of illness. It is recommended to evaluate cost of illness, quality of life and cardiological risk factors together, because of there is a statistically significant difference between these issues in the future studies.
