The relationship between disease activity and IL-6 and CRP levels in patients with acute pancreatitis

Abstract (EN)
Introduction and AimAcute pancreatit (AP) is the activation of enzymes in pancreas and its digest by pancreatic tissue. In acute pancreatit in order to determine the clinical severity and medical prognosis various scoring systems and inflammation madiators are used. In this study our aim is to search whether the comparisons between Ranson, APACHE and Balthazar scoring and inflammatory madiators such as CRP and IL-6 are statistically meaningful or not.Material and MethodBetween the dates of 2009 and 2010 by consulting the emergency service and gastroenterology polyclinics of Dicle University Medical Faculty Training and Research Hospital, 52 (36 women, 16 men, average age 55,7; distrubution 17-86) patients, who were diagnosed acute pancreatit and treated in gastroenterology polyclinics, were included in this study.Results36 of the patients? diagnosed with acute pankreatit were women,16 were men and their ages were between 17 and 86, the average age were 54,4 in women and 59,9?in men. All patients? avarage age was 55,7. When etiology of acute pankreatit is examined; in 30 of the patients biliary reasons, 1 one of them hypertriglyceridemia, 2 of them post-ERCP were determined and in 20 patients it was evaluated as idiopathic.According to the correlation between AP scorring and inflammatory mediators it was found meaningful as (P<0,001) among CRP, Ranson score and Ranson score. Between Ranson score and Interlokin 6 (P:0,014) there was a meaningful correlation.Patients were divided into two groups as mild and severe AP. According to this grouping it was determined that the differentiation of APACHE 2, Ranson and CRP levels as mild and severe is meaningful with duration IL-6 of hospital stay, age groups and blood ürea nitrogen (BUN). (P levels respectively P=0,03, P=0,02, P<0,001, P=0,03)The patients evaluated as severe AP who scoring according to Balthazar clacification the score point increased; Ranson, APACHE 2 scores with CRP, interlokin levels raise was dertemined. (P<0.001)was found meaningful statistically.ConclusionRanson, APACHE II, Balthazar scorrings, IL-6 which is earlier indicator of inflamation that are used to determine the severity of acute pankreatit and CRP measurements which increases later than IL-6 were found meaningful both alone and comparisons.
