The effects of helicobacter pylori eradication on insulin resistance, beta cell function, leptin levels and other metabolic parameters

Objective: We aimed to evaluate the metabolic status of prediabetic patients with Helicobacter pylori infection and to determine the changes in the biochemical parameters after the treatment of Helicobacter Pylori infection. Material and methods: These prospective study was performed over 36 Helicobacter pylori positive volunteers who have insulin resistance. We used HOMA-IR to verify insulin resistance. We planned to use some clinical and laboratory data (such as body-mass index, HOMA-IR, lipid profile, CRP, leptin) to evaluate them. Volunteers positive for Helicobacter Pylori were given eradication treatment (Clarithromycin, Amoxicillin/Clavulanate, Lansoprazole) for two weeks. Patients re-evaluated 16 weeks after the completion of the treatment with same parameters which were assessed before the treatment. Results: Patients were divided into two groups according to treatment success. The success of treatment in 19 patients were positive and 17 patients were negative. The baseline characteristics of both groups were similar. In successful treatment group, a statistically significant decrease was noted in HOMA-IR (p=0,0001) and body-mass index (p=0,002) after the treatment. In the other group, there were no significant changes at these two parameters. In both groups, there were no significant changes in other parameters (lipid profile, CRP, leptin) between the pre and post treatment assessments (p>0.05). Conclusion: In this study, we have shown that a positive effect on insülin resistance can be achieved by Helicobacter pylori eradication treatment. Although insulin resistance decreased with the eradication of Helicobacter pylori, it is not possible to state the role of Helicobacter pylori in diabetes since there were no changes in other parameters; thus, advanced studies are needed.
