Oxidant status, antioxidant status, and irisin levels in prematured babies of women with choryoamnionitis and extended preterm early membrane rupturing

Abstract (EN)

Objective: The failure of labor to begin despite the rupture of the amniotic membrane for 18 hours is called prolonged premature rupture of membranes (PROM). The occurrence of this action before 37 weeks of gestation is called prolonged preterm premature rupture of membranes (PPROM). Infection in the placenta and / or decidua and amniotic fluid is called chorioamnionitis. These conditions can cause serious morbidity in perinatal and postnatal period in newborns. In our study, it was aimed to compare the oxidant-antioxidant levels in the premature babies born from mothers with prolonged PPROM and chorioamnionitis and to compare them with the control group. Material and method: Our study was conducted prospectively within the Pediatric Intensive Care Units of the Dicle University Faculty of Medicine, Department of Neonataology. Our study consists of 90 cases whose gestational week is ≤32 weeks and less than 1500 grams in the last year. Our patients are composed of patients with chorioamnionitis (29 patient group), prolonged PPROM (31 patient group) and not diagnosed with chorioamnionitis or prolenged PPROM (control group, 30 patients). Demographic data and laboratory parameters of all patients were recorded prospectively and used in our study. Results: Necrotizing enterocolitis (NEC) was found to be statistically significantly higher in the control group. C-reactive protein and procalcitonin levels were found to be significantly higher in patients with chorioamnionitis. TAS levels were positively correlated with WBC (white blood cell) and neutrophils in patients with chorioamnionitis. In patients with PPROM, irisin levels were negatively correlated with WBC and lymphocyte. However, irisin level was found to be higher in patients with PDA in the PPROM group than in patients without PDA. In the control group, irisin levels were found to be higher in those who developed NEC than those who did not. At the same time, irisin and TAS levels were found to be lower in patients who developed BPD. Conclusion: It is interesting that TAS levels increase like a positive acute phase reactant (AFR) in inflammatory events and irisin levels are at low levels in inflammatory events. An answer should be sought to the question of whether there is inflammation because irisin falls, or whether irisin falls because of inflammation. The association of high irisin levels with patent ductus arteriosus is worth investigating.The low irisin levels, especially in patients who develop BPD, support the claims that irisin can prevent pulmonary damage, and it is worth doing more studies.
