Abstract (EN)

Aim: Our aim in this retrospective study is to investigate the effect of sarcopenia on early mortality and survival in patients operated for peptic ulcer perforation in our clinic. Materials and Methods : Between January 2010 and December 2022, a total of 238 patients over the age of 18, who had no malignancy, had computerized tomography imaging, and underwent emergency surgery for peptic ulcer perforation using simple closure in Mersin University Medical Faculty Hospital, were included in the study. The patients were divided into two groups as sarcopenic and non-sarcopenic with the Hounsfield Unit Avarage Calculation (HUAC) data obtained from preoperative computerized tomography imaging. Moreover; after dividing them into groups according to gender, age and body mass index characteristics, the effects of these factors on early mortality and survival were examined. Results: While the age of patients with early mortality was 65.5 years, the age of patients without mortality was lower and 49.2 years. According to this result, age was found to be significant in its effect on early mortality. (p<0.05) According to HUAC 59 of the patients were evaluated as sarcopenic, while 179 of them were non-sarcopenic. While the rate of sarcopenia was 54.5% in patients with early mortality, this rate was 21.8% in patients without early mortality. İn the analysis of the survival evaluation; while the age was 63.6 years in patients with mortality during the follow-up period, it was lower and 48.4 years in patients who were still alive during at least 6 months of follow-up. According to this result, there was a significant relationship between survival and age. (p<0.05). While the rate of sarcopenic patients is 56.8 in patients with mortality in long-term follow-up, this rate is 18.9 in patients without mortality. There was a significant relationship between sarcopenic status and survival (p<0.05). Conclusion: It was determined that advanced age and sarcopenic state increased early mortality and decreased survival in peptic ulcer perforation.
