Perioperative assessment of the body water composition via bioimpedance spectroscopy in pediatric patients with congenital heart defect who underwent open heart surgery

Abstract (EN)

Objective : The assessment of alterations at body water composition in perioperative period are significant morbidity and mortality factors for pediatric patients with congenital heart defect and it is also important for costs during hospital stays . Actual methods for intensive care unit evalution are not adequate . In this study , patients with congenital heart defect who underwent open heart surgery , the effectiveness of assessing perioperative body composition via bioimpedance spectroscopy ( BIS ) with body composition monitor ( BCM ) is reliable , cost-effective and relatively simple to perform in according to the other monitorization methods as clinically , transthoracic echocardiogram and invazive monitorization was investigated. Material and Method : Twenty patients with congenital heart defect who underwent open heart surgery were compared with twenty healthy controls , both of two groups were under ten years old , prospectively enrolled in this study from July 2015 through October 2015 in the Clinics of Cardiovascular Surgery, İstanbul Bilim University Faculty of Medicine. BIS was evaluated wih transthoracic echocardiogram , clinically and invazive monitorization. The data about volume status with BIS method was compared to clinically , transthoracic echocardiogram and invazive monitorization . Patients with permanent pace maker and systemic organ dysfunction except heart defects were not included . Statistical analysis was made using SPSS for Windows 11.5 Results : Eight patients were female and twelve patients were male in patient group. Seven were female and thirteen were male in healthy controls . Mean age was 21,10 ± 28,03 months , mean height was 75,15 ±21,78cm , mean weight was 9,12 ±5,81 kg , mean body surface area was 0,41± 0,19m2 , mean body mass index was 14,61±2,07 kg/m2. There is no statistically significant difference between the patient and control group demographic findings. There is statistically significant difference between the parameters of BIS ECW and transthoracic echocardiogram VCI (p<0,05). There is statistically significant difference between the parameters of BIS E/I and volume balance . There is no statistically significant correlation about the other parameters. When we compare the preoperative and postoperative findings between BIS , transthoracic echocardiogram and CVP values , there is statistically significant difference between the parameters of TE/TA , OH , ROH ve E/I (p<0,05) but there is no statistically significant difference between the parameters of CVP . We determine the sensitivity and specificity of BIS parameters for assessing the overhydration is %70,58 and %58,8 . Conclusions : Our data suggest that BIS is reliable , cost-effective and relatively simple to determine perioperative volum status and body water composition in operation room and intensive care unit when it's used with the other monitorization methods like clinically , transthoracic echocardiogram and invazive monitorization about the pediatric patients with congenital heart defect who underwent open heart surgery Keywords : congenital heart defect , bioimpedance , body water composition , BIS
