Abstract (EN)

Study Aims: The aim of this study was to comparison of radiological classifications (Atlanta and Balthazar) with Ranson criteria in the diagnosis of acute pancreatitis severity and prognosis. Materials and Methods: 80 patientswho underwent MDCT scan for diagnosis of acute pancreatitis in Radiology department between January 2010 and December 2013 were retrospectively reviewed. Patient demographics, clinical, laboratory and radiological findings, Ranson criteria were analyzed retrospectively. MDCT classifi-cations Atlanta and Balthazar, clinical Ranson criteria were defined. Fordetermina-tion of acute pancreatitis; severity severe cases and for prognosis; mortality and mor-bidity were compared with Atlanta and Balthazar classifications and Ranson criteria. In Ranson criteria, severe cases were defined using scoring system; in Atlanta and Balthazar, severe cases were defined according to necrosis degree in CT. Morbidity was defined as the duration of hospitalization. Results: The study included 80 patients who were diagnosed with acute pancreatitis. 24 patients (30%) were male and 56 (70%) were women between ages 21 and 86 (mean 57.15 ± 7.13). When subjects were evaluated as mild and severe; according to Atlanta classsification, 59 (74%) cases were mild and 21 (26%) subjects were defined as severe pancreatitis. According to Balthazar classification, 62 (77.5%) cases were mild pancreatitis, 18 (22.5%) cases were severe pancreatitis. Accordingto Ranson criteria 68 (85%) cases were mild pancreatitis, 12 (15%) cases were severe panc-reatitis. In cases with severe pancreatitis, Atlanta and Balthazar classification were correlated (p<0.001), as opposed to no significant relationship between Atlanta clas-sification and Ranson criteria (p=0.118). No correlation was found between Balthazar classification and Ranson criteria (p=0.64). No statistical analysis was done for mortality rate as the number of cases was very low and did not reach the statistical significance. In terms of morbidity, Atlanta classification and Ranson criteria were correlated (p<0.001) as well as Balthazar classification and morbidity ra-tes.(p=0.002). However, no correlation was found between Ranson criteria and mor-bidityrates (p=0.264). Conclusions: We compare Atlanta and Balthazar classifications with Ranson criteria in the diagnosis of the acute pancreatitis severity and prognosis. Atlanta and Baltha-zar classification correlation was significant. Ranson criteria wasn't correlated with the two classifications. Acording to comparison with morbidity; Atlanta classification was better than Balthazar classification and Ranson criteria. Therefore, in the diagnosis of acute pancreatitis severity and prognosis; it is better to use Atlanta clas-sification.
