Abstract (EN)

ABSTRACT THE EFFECT OF INJECTABLE PLATELET-RICH FIBRIN (I-PRF) APPLIED TO THE FLAP DISSECTION PLANE IN SEPTORHINOPLASTY SURGERY ON POSTOPERATIVE EDEMA, ECCHYMOSIS, AND PAIN Objective: Platelet-rich fibrin (PRF) is a second-generation platelet concentrate. This autologous biomaterial, first introduced in the early 2000s, has gained increasing popularity due to its positive effects on wound healing as well as its hemostatic and anti-inflammatory properties. The aim of this study was to evaluate the effect of injectable platelet-rich fibrin (I-PRF) on postoperative periorbital edema, ecchymosis, and pain following septorhinoplasty surgery. Method: A total of 90 patients were included in the study, consisting of 45 patients in the study group and 45 patients in the control group. 20 cc of venous blood was drawn from the patient. I-PRF was obtained from this blood by centrifugation at 700 rpm for 3 minutes. I-PRF was applied topically to the entire dissection plane via a syringe. The study and control groups were evaluated at 0 hours (before taping and splinting), 1 hour, 4 hours, 1 day, 2 day, 5 day, 7 day, 10 day, and 15 days postoperatively, and scored according to edema, ecchymosis, and pain scoring systems. Results: Compared with the control group, the I-PRF study group demonstrated significantly less ecchymosis on postoperative days 1 and 2. Similarly, significantly lower edema scores were observed in the study group on postoperative days 1 and 2. However, no statistically significant difference in pain scores was found between the two groups at any of the evaluated time points. Conclusion: In septorhinoplasty patients, I-PRF application significantly reduces postoperative edema and ecchymosis during the first and second postoperative days. However, no significant effect of I-PRF on postoperative pain was observed. Keywords: septorhinoplasty, rhinoplasty, edema, ecchymosis, pain, platelet-rich fibrin.
