Abstract (EN)
Objective: We planned to analyze the relationship between demographic data, biochemical parameters, echocardiographic parameters and other variables of patients hospitalized in the intensive care unit with the diagnosis of diastolic heart failure and diagnosed with pneumonia in our center and present them as a center's experience. Materials and Methods: Study; Itwasperformed on 63 people (in patients with diastolic insufficiency and pneumonia) between the ages of 18-93 at Gaziantep University Şahinbey Training and Research Hospital between 01.03.2019 and 01.03.2020. The conformity of the numerical variables to the normal distribution was tested with the ShaphiroWilk test. Mann Whitney U test was used to compare the non-normally distributed variables in two groups. Kaplan Meier method was used to calculate survival times. SPSS 22.0 Windows version package program wasused in the analysis. P<0.05 was considered significant. Results: The median age of the patients was 70, and 54% (35) were male and 46% (29) were female. Hypertension (HT) was found in 31.7% of the study group, diabetes mellitus (DM) in 25.4%, and coronary artery disease (CAD) in 36.5% of the study group. A significant correlation was not found between the patients who were alive and the height of LVEDD and LVESD (p:0.215), (p:0.229). A significant correlation was found between not receiving inotropic treatment and low SMARTCOP score (p:0.015). In patients without CAD, the A' wave velocity was high and the E/A ratio was low (p:0.027), (p:0.046), a weak negative correlation was observed between the Acute Physiology and Chronic Health Assessment 2 score (APACHE2) and LVEDD (r:- 0.295), (p:0.019). Mean survival at 1 month was 54.8%, and 1-year survival was 30.6%. Conclusion: Considering the general results, no correlation was found between the presence of DM, proBNP level and hospital stay in patients and diastolic heart failure parameters. A significant correlation was found between the survival of the patients and the increase in the E deceleration time and the increase in the speed of the A wave (p:0.018), (p:0.007). Since the E-deceleration time is shortened in the restrictive type of diastolic filling and abnormal relaxation, prolongation of this time is associated with survival. A significant correlation was found between the increase in septal S' wave velocity and the E/A ratio and the absence of intubation (p:0.007), (p:0.012). No significant correlation was found between diastolic heart failure and echocardiographic parameters in the patients. Keywords: Pneumonia, diastolicdysfunction, echocardiography, SMARTCOP score
