Abstract (EN)
This study was planned to investigate the effect of preoperative nutritional support on postoperative morbidity, mortality, and length of stay in patients planned to undergo surgery for non-small cell lung cancer (NSCLC). The study was carried out on 181 patients between January 1 - December 31, 2017 in Yedikule Chest Diseases and Chest Surgery Training and Research Hospital. 81.2% of the patients were male, mean age was 59.74±8.9 years. The mean BMI was 25.33±3.87kg/m², and the mean lean body mass was 56.86±8.2kg. 52.5% of the patientshad NRS 2002 score of 2; %30,4 had 3, 12.7% had 4, 4,4% had 5.In 38.1% of the patients complications occurred; major complication rate was 18.8%. Mean hospitalization time was 7,67±5,87 days. 62.5%(n=5) of score 5 patients developed complications, and 39.1%(n=9), 21.8%(n=12) and 45.3%(n=43), respectively of score 4, 3 and 2(p=0.016). Complication rate was found to be 7.2 times for DM (p=0,001), 5.1 times for chest wall resection (p=0,003), 2.7 times for score 2 with no nutritional support (p=0,002) and 8.7 times for score 5(p=0,02). Pneumonectomy, chest wall resection, non-supportive care increased development of major complication 4.6 times, 7.8 times, 10.1 times respectively(p=0,001 p=0,003 p=0,003). Finally; in patients receiving nutritional support, incidence of complications were significantly reduced, hospitalization time was shortened, and loss of albumin was less in the first postoperative month. Factors affecting development of major complications, only nutritional support can be regulated by the clinician. Therefore, we recommend patients who undergo extended lung resection should receive supportive treatment, even though their pre-evaluation tests and nutrition level are adequate.
