Abstract (EN)
The Effects of Using the Web-Based Double Eye Control Program on Medication of High Risk Drugs in Pediatric Patients Aim: In this study, it was aimed to examine the effect of using the Web Based Double Eye Control Program as a strategy of standardization and preventing medication errors during high risk drug applications of pediatric nurses. Materials and Methods: The study was carried out as a semi-experimental study by using one group posttest pretest design between the dates of November 2016 and June 2018 in the Pediatric Surgery, Pediatric Surgery Intensive Care and Neonatal Surgery Intensive Care Clinics of Ege University's Medical Faculty where 0-18 years old patients were hospitalized. The sample of the study consisted of 24 nurses working in these clinics, taking part in drug applications and accepting to participate in the study, and all of the high risk drugs adminestered by the nurses during the observation period of the researcher (532 drug doses applied before the intervention and 538 drug doses applied after the intervention, 1070 drug doses applied in total). The data were collected by using the Personal Information Form for Nurses' Characteristics and the Drug Application Observation Form. The medication error rates were determined by observation method at first stage. After that, the Web Based Double Eye Control Program was started to be applied; the medication error rates were determined by postprocedural observation method. The required permissions were obtained to conduct the study. Results: As a result of the observational study of the errors in the application process of the nurses, according to deviations from 10 correct rules in Drug Applications and The Application Steps of the Web Based Double Eye Control Program, when the total error rate was examined, the preprocedural medication error rate was 47.8%; the postprocedural medication error rate was 19.4%. The medication error rates were evaluated according to working hours. When the error average rates according to the drug application steps were examined, the prepocedural preparation error average rate was 34.9% while the postprocedural preparation error average rate was 10.8%. The preprocedural administration error average rate was 25.6%; the postprocedural administration error average rate was 10.3%. The preprocedural follow-up error average rate was 61.5%; the postprocedural follow-up error average rate was 22.6%. The preprocedural and postprocedural recording error average rates were 69% and 34%, respectively. The preprocedural medication error rate was 97.6%, the postprocedural medication error rate was 39.5% on the 08:00-16:00 shift. The preprocedural medication error rate was 100%, the postprocedural medication error rate was 82.1% on the 16:00-08:00 shift. The medication error rates were also evaluated according to the route of administration. For intravenous medications, the preprocedural and postprocedural medication error rates were 97.8% and 55.8%, respectively. For medications through feeding tubes, the preprocedural and postprocedural medication error rates were 100% and 80.8%, respectively. For medications through oral route, the preprocedural and postprocedural medication error rates were 100% and 60%, respectively. For medications through respiratory tract, the preprocedural and postprocedural medication error rates were 100% and 58.8%, respectively. It was determined that the decrease in the error rates was statistically significant (p = .000). Wrong patient error and wrong drug error were not observed before and after the intervention. Conclusion: It was seen that the nurses' medication error rates decreased by using the Web Based Double Eye Control Program developed by the researcher. This finding suggests that the program is effective and significant. Key words: medication error, high-risk drugs, pediatric nursing
