Anti-hbc seropositivity patients who have hepatosteatosis and the relationship between the level of serum adipokine

ABSTRACTThe fact that both hepatostatosis and the rate of HBV exposure are common in our population suggests that these two entities may be related to each other in the light of the literature. The frequency of cryptogenic cirrhosis is still about 16-20% among the other causes of cirrhosis. Considering that hepatosteatosis causes about 50% of cryptogenic cirrhosis, preventing hepatosteatosis may be conceived as preventing the development of cirrhosis. Although many trails have demonstrated steatogenic effect of HBV, it is unclear whether the positivity of anti-HBc IgG results in hepatosteatosis, and even if so, its role in the development of hepatosteatosis.The aim of this study is to investigate the relationship between insulin resistance and serum adipokines levels in individuals who previously exposed to hepatitis B.USG-determined 80 patients with hepatosteatosis are included in this study, with detailed physical examination and the medical history. Biochemical parameters include lipid profile and transaminases as well as fasting blood glucose and insulin levels. Using fasting blood glucose and insulin levels, HOMA-IR (Homeostasis Model Assessment-Insulin resistance) was calculated as follows;(HOMA-IR = Fasting insulin value (?IU/ml) x Fasting glucose value (mg/dL)/405)HOMA-IR > 2,7 was described as insulin resistance, and these patients was investigated for anti-HBc IgG positivity. Anti-HBc IgG value was analyzed by ELISA (Enyzme Linked Immuno Sorbent Assay). Level of adiponectin, resistin and leptin in serum sample were analyzed in cases with Anti-HBc IgG (+) and anti-HBc IgG (-). While the level of leptin in anti-HBc IgG (+) group was 31569,72 ±14027,64, it is detected as 25410,73±10978,26 in anti-HBc IgG (-) group. The levels of leptin in anti-HBc IgG (+) and anti-HBc IgG (-) group are found statistically significant, 31569,72 ±14027,64 and 25410,73±10978,26 respectively, p=0,047. But there was no difference between two groups in the level of adiponectin and resistin. Exposed to hepatitis B previously, in individuals with hepatosteatosis determined by USG, high level of leptin may be associated with NAFL, and anti-HBc IgG positivity should be taken into consideration in this context.Key words: Non alcoholic fatty liver (NAFLD), HBV, Insulin resistance, Hepatosteatosis
